New Patients
Once you have scheduled your new patient appointment, please complete our New Patient forms to ensure your provider has all the necessary information for your appointment!
Returning Patients
If you are an established patient, you will receive a link to your Medent Pre-Check-In via text or email 5 days prior to your appointment. Here, you will update your personal information, HIPAA preferences, sign important consents, and fill out your pre-visit questionnaire.
Helpful Tips:
- In order to receive your link via text, we must have your cell phone provider information. Not sure if we have that info? Please contact our office via call, text, or email!
- If you cannot find your link in your email, please check your spam folders.
- You may need to enter your zip code to verify your identity, so please make sure we have your current home address.
