New Patients
Once you have scheduled your new patient appointment, please complete our New Patient forms to ensure your provider has all the necessary information for your appointment!
Re-Establishing Patients
If it has been 6 to 24 months since your last visit, please complete our Restart Patient forms before your appointment. This helps our providers review your updated health information and prepare for your visit.
Returning Patients
Effective April 1, OVYVO will no longer use Medent Pre-Check-In forms for follow-up visits.
Instead, established patients will receive their appointment reminder by text or email through HealthBanks. That reminder will now serve two purposes:
- Confirming your appointment
- Accessing your pre-visit form
To complete your pre-visit form, please first click to confirm your appointment in the reminder message, then use the included link to fill out your form.
Patients may complete the pre-visit form as early as 7 days before the appointment, but we ask that it be completed at least 48 hours in advance whenever possible. Forms submitted less than 48 hours before the appointment will still be accepted, but they may not allow enough time for our team to fully update your chart before your visit.

- Please keep your appointment reminder text or email, as you will need it to confirm or cancel your appointment and to access your pre-visit form.
- The pre-visit form link will be the same each month, and you can always find it on this page.
- Completing your form earlier helps our team prepare more thoroughly for your visit.
- If you do not see your reminder email, please check your spam or junk folder.
